Certificate of Insurance Request

General Liability Coverage Available to all USA Clubs

If your venue requires proof of insurance, we can support USA-based Captains and Clubs to make sure you're covered each Season you play.

As part of K9 Frisbee Toss & Fetch League’s  Sport & Recreation Insurance Policy coverage, any US-based Captain may request a Commercial General Liability Certificate of Insurance (CoA) to provide to their local venue, when required.


How to Request a Certificate of Insurance

The process for Club Captains (or designated Co-Captain) to request a Commercial General Liability Certificate of Insurance that names the additional insured person(s) or organization is as follows:

  1. Download a PDF of the K9 Frisbee Toss & Fetch Liability Waiver & Publicity Release. Print this release and ensure that all Club members read, sign and date the release. Keep this release for your records.


  2. Complete and submit the following form to request a Commercial General Liability Certificate of Insurance (CoA). ALL FIELDS ARE REQUIRED.

NOTE: CoA’s are valid for 12 months beginning annually on June 1 and expiring on June 1.
